His team delivered a technically perfect OEE dashboard for a production line. Six to nine months later, every terminal was shut off. The postmortem surfaced two missed details. Maintenance was never folded into the design, and a single failed photo eye broke throughput calculations with no manual reconciliation path, which destroyed operator trust in the data. The second miss was behavioral.
