And when does degeneration go beyond what we'd expect for age alone? So today I'm gonna break down the medical science behind musculoskeletal degeneration and how clinicians determine whether something is typical aging or potentially accelerated by mechanical stress or injury."

      "body": "First, what is normal degeneration? So as we age, our bodies change and that's not controversial, like that's just a fact. So for example, intervertebral disc gradually lose hydration, cartilage thins over time, joint space may narrow, bone density is gonna shift. This process is gradual and typically it's gonna be symmetrical. In many people, mild degenerative changes begin appearing on imaging as early as their thirties or forties, even without symptoms."

      "body": "So yes, aging does contribute to musculoskeletal change. Like that's just, it happens, but that's not the whole story, right? Especially in our veteran population. Aging versus mechanical stress. Here's where nuance matters."

      "body": "Normal aging follows predictable biological curves. So accelerated degeneration often follows mechanical patterns. Examples like the spine. If someone has repetitive axial loading, heavy load carriage, airborne operations, so you're rucking, you're doing airborne ops with full combat equipment, you have vibration exposure, maybe you're a pilot, right? And G forces are being applied, repetitive trauma."

      "body": "You might see things like earlier disc height loss, focal degeneration at high stress levels. Okay, listen, when I went in the army, I was sixty eight inches tall. I was five'eight. By the halfway through my career, was sixty seven inches. And I'm convinced that, I mean, and this is multiple measurements over time."

      "body": "And I'm convinced that it has to do with all of my airborne operations, rough marching, all the things that we do, right? So focal degeneration at high stress levels, end plate changes happen, facet joint hypertrophy. You guys might see all that on your CT scans or your MRIs or whatever. That pattern may not mirror generalized aging. The location and severity matter."

      "body": "So joint degeneration is not always symmetrical either. Age related osteoarthritis often develops gradually and symmetrically, but post traumatic degeneration frequently appears on one side or worse on one side in a specific joint at a site of prior injury. An example, okay? Here's an example. A veteran with a documented ACL tear in service develops early, like very early medial compartment knee arthritis, joint space narrowing, disproportionate to their age, mechanical instability."

      "body": "That's different from diffuse age related wear and wear, okay? History and imaging patterns matter. Biomechanics change everything. The body is a kinetic chain. We've done videos on kinetic chain before."

      "body": "So if one joint is injured or unstable, it can alter the load distribution elsewhere. For example, knee instability can alter hip mechanics. Ankle injuries can affect the gait, lumbar spine issues can impact the pelvic tilt. Over time, altered biomechanics can accelerate degeneration in adjacent structures. That's not just aging, that's stress redistribution."

When evaluating whether something is just age related, medical professionals consider things like age at symptom onset, imaging severity relative to age, focal versus generalized changes, prior imaging documentation, occupational or mechanical stress history, symptom progression timeline. So a thirty eight year old with advanced disc degeneration at two levels after documented trauma is different from a 65 year old with mild diffuse changes. Okay? So context matters."

      "body": "So it's important to keep in mind that not all degeneration is service related. Let's stay balanced y'all for real. There might be a combination going on. Degenerative changes are common in the general population. Many people develop arthritis without trauma."

The key question isn't is degeneration present, it's is the pattern, timing and severity consistent with expected aging or is there evidence of acceleration? That requires individualized analysis.       "body": "So let's talk about imaging because there's some imaging traps as well. So imaging findings alone don't tell the whole story. Some people have severe imaging findings with minimal pain. Others have significant pain with mild imaging changes. Clinicians evaluate things like clinical symptoms, functional impairment, physical exam findings, imaging correlation."

      "body": "That's why VA looks at range of motion as well because that's like how are you functioning? So timelines, imaging supports a diagnosis. It doesn't define the entire clinical picture. Bigger picture is that when someone says, Hey, it's just age. What they may really mean is these changes are commonly seen in people over a certain age, but common does not mean it's inevitable."
