So one of the biggest misconceptions in medicine is that pain is purely mechanical. That if somebody looks bad on imaging, it hurts a lot."

      "body": "And if imaging looks mild, it shouldn't hurt that much. But pain is not just about structure. Pain is processed in the nervous system. So your brain interprets signals coming from tissues, and that interpretation is influenced by many factors. So research has shown this clearly."

      "body": "Right? Large studies have shown that many people with things like herniated discs, degenerative disc disease, rotator cuff tears, knee arthritis, labral tears. People can have no pain at all with those things. Right? None at all."

      "body": "And at the same time, some people with relatively mild imaging findings experience severe persistent pain. So, clearly, imaging alone does not determine symptom severity. Why that happens? K. Why does that happen in the nervous system?"

      "body": "When you experience injury, inflammation, or trauma, the nervous system becomes activated. In acute injury, that activation can become protective. But in some cases, especially after repeated injury or prolonged stress, the nervous system becomes, like, desensitized or sensitized. This is called central sensitization. Right?"

      "body": "So it means the brain and the spinal cord become responsive to pain signals. The threshold can lower. Right? Signals can amplify. Even minor stimuli can feel really intense."

      "body": "This is not just physiologic. It is a neurophysiological issue. So pain is an output of the brain. Pain is a protective response. The brain evaluates things like tissue signals, past injury history, emotional stress, sleep quality, inflammation, overall threat perception."

      "body": "And then it generates a plan. K? It generates pain. It can generate, whatever it needs to to help protect you. Right?"

      "body": "So if the nervous system has been repeatedly activated with things like chronic musculoskeletal strain, combat exposure, PTSD, repeated microtraumas, poor sleep, the pain response can become persistent even if the structural damage appears mild. Right? It's multifactorial. Structural findings don't equal pain severity. So let's take disc degeneration as an example."

      "body": "Some individuals have severe disc height loss and minimal symptoms. Others have mild bulging and severe pain. Why? Well, that is because nerve involvement matters. Inflammatory response matters."

      "body": "Muscle guarding matters. Pain pathway sensitization matters. Imaging shows anatomy. It does not show nervous system amplification. So let's talk about the role of inflammation."

      "body": "Inflammation can irritate nerve endings. Even subtle inflammatory changes are not easily visible on imaging, and they can cause significant pain. Additionally, chronic stress and sleep disruption can increase systemic inflammation. So someone with PTSD or chronic step stress might have a heightened inflammatory state that amplifies pain perception. Again, that doesn't mean that pain is imagined."

      "body": "It means that the nervous system is highly reactive. So chronic pain changes the brain. When pain becomes chronic, the brain can actually reorganize around it. So neural pathways can strengthen. Pain processing areas can become more active."

      "body": "This is called neuroplastic change. It's been documented in chronic back pain, migraines, and neuropathic conditions. So pain can persist even after the original injury stabilizes. That doesn't make it less real. It makes it neurobiologically entrenched."

      "body": "Right? So important, this can go both ways. So let's just stay balanced. Sometimes imaging findings are severe and clearly correlate with symptoms. Sometimes mild findings are truly incidental."
