The Main Story: The Trap of the "Open Door Policy" and Weaponized Feedback Today, we are talking about the biggest lie currently circulating in Corporate America: the mythical "Open Door Policy". We pull back the curtain on how organizations claim to want a speak-up culture but actively punish employees who bring up actual, systemic safety feedback. Real learning organizations thrive on bad news to fix failing systems, but toxic systems treat bad news like a virus and weaponize it against the worker. If you are a leader, stop pretending you want the truth if you're only willing to accept compliance.

Police flooded the area with emergency crews, shut down nearby roads and parts of Interstate 805, and urged the public to stay away while- the scene was being secured. By late afternoon, the authorities said that it had been neutralized. Take a listen to what happened. At about 1143, we received a call of an active shooter at the Islamic Center. Within four minutes, officers arrived on scene and observed immediately three deceased, what They immediately began to deploy with an active shooter response into the mosque and adjacent school. At about the same time, we began to receive calls from just a couple blocks away that we had more active gunfire. Officers were also responding to that call for service at the same time. The person who shot the man in the head was called to the scene, where they found a vehicle in the middle of the street, with who we believe to be the shooters in this incident, both deceased. That's part of what exactly was going on based on the reports right there in real time. Scott on how people died, take a listen to what he had to say as well. How many total deceased? So we have our two suspects that are deceased. We have three victims at the Islamic Center. All three are adults for a total of five people that are deceased. to this incident. So a lot of deceased in regards of what is going on is you can definitely take a listen there so some interesting stuff as we are talking no doubt about that. So let's start talking about this one because this one was kind of one of those things that came out of left field yesterday as well. So let's talk Mark Furman. The former Los Angeles detective, tied forever to OJ Simpson's murder trial, has died at the age of 74, battling with an aggressive form of throat cancer. Furman became one of the most recognized and controversial figures in American criminal justice history after helping investigators, or investigate better saying, the 1990s. before murder of Nicole Brown and Ron Goldman. He was among the first detectives on the scene and as a leader testified in the national televised murder trial of O.J. Simpson. The case exploded into media phenomenon and became a major flashpoint in debates about race, policing, and the justice system. Furman, Credibility took a. a massive hit during the trial, recording surface of him using racial slurs, contradicting earlier testimony in which he claimed that he had not used such language in years. He later pleaded no contest to a felony perjury charge connected to his testimony. Furman strongly denied accusations that he planted evidence, including the infamous bloody glove discovered. Simpson's estate. One of the most scrutinized evidence in modern legal history, the courtroom moment where Simpson struggled to put the gloves on became one of the defining images of the trial. After retiring from the LAPD in 1995, Furman reinvented himself as a conservative radio host and true crime author, writing books including The Murder in Brentwood about Simpson's case. Take a listen to what he had to say when he was in trial. Most of these notes were what Officer Risky was pointing out to Detective Phillips and myself and what I observed. What did you intend to do? So were these notes rough notes? Yes. notes later on? I use them to go back to these areas and use them as a guide and what to go back to and and prioritize them. It's kind of interesting I mean thinking about this years and years later on how this guy became super popular all of a sudden and then when he took the fifth because that's what occurred when he the fifth when he was on the stand people were like uh-oh some things going on because automatically you know it became one of those things of he's guilty because he took the fifth i don't know if you remember that i mean that was years and years ago i mean mid-90s at this point who truly cares what this guy has to say who gives a shit safety show. Okay, I agree. Who cares, man? Who cares what I have to say? Okay, so let's continue talking about some other things that are going on inside of the multiverse. Okay, so where do we go from here? And do we really care? I guess is the other question. God have mercy is kind of the way that it goes, I think is the remainder of that thing. have mercy on myself as we are talking. Okay, so as we take a look around and go into the whole kit and caboodle, because there's just no other way to put it. Donald Trump and Bettina Anderson are reportedly set. Excuse me, Donald Trump Jr. and Bettina Anderson are reportedly set to get married over Memorial Weekend in the Bahamas. According According to reports, the couple plan to hold a small private ceremony on the island with only close family and friends attending. The two reportedly have been considering a much bigger White House wedding, but the source say the concerns over how the lavish event might look during the ongoing war in Iran caused them to scale things back. Instead of postponing the wedding entirely, They reportedly decided to keep it intimate and low-key. Well, as low-key as a private island in the Bahamas can be, I guess, is really the way to think about it, anderson already celebrated with her enchanted garden-themed bridal shower at Mar-a-Lago in April. Guests reportedly included members of the Trump family, like Ivanka Trump, Laura Trump, Tiffany Trump, and Marla Manson. Maples along with social lights in the book Palm Beach insiders. Trump's juniors daughter Kai Trump also attended. So there you go. There you go. Some things to talk about. So let's talk about this one, because this is definitely going to be one of those things that as we do reference this, it's like, wow, it's already been that long, and you kind of get some of it. But Nancy Gunthery has now been missing for 100 days after authorities say that she was kidnapped from Tucson, Arizona, home back in February. The first investigators believe that the 84 year old was taken against her will, and surveillance footage later showed a masked man attempting or tampering, better saying with the doorbell camera overnight before. she disappeared. No suspects have been identified. Chris Nanos, the chief leading investigator, says that he is no longer personally communicating with Nancy's family. Instead, the contact is now handled through the FBI. Earlier in the case, Nanos had been texting occasionally, speaking directly with but says investigators are now handling all update communications so there you go there you go what do you think here hundred days I mean it seems like such a long time it's like in that case we were talking about it for such a such a while and then it was like talk talk talk talk talk and then it was gone so I don't know I mean it's just one of those things that I look at and go, man, this is a kind of a weird one when we start thinking about it when it's all said and done. No doubt about that. So there you go. Take a listen to this one. So this one was becoming popular over the over the last week and a half or so. So let's start talking about this one. A federal jury in California has rejected Elon Musk's lawsuit against open AI dealing with With a major blow to his claims that the company abandoned its non-profit roots in favor of chasing profits with backing from Microsoft, Musk sued OpenAI in 2024, accusing CEO Sam Altman and co-founder Greg Brockman of violating the organization's regional mission to develop AI for benefit. of humanitarian rather than for corporate gain. He claimed that OpenAI had essentially transformed from a charity-minded non-profit into a profit machine tied closely to Microsoft but after three weeks trial in federal court in Oakland, a nine-member jury unanimously ruled that Musk waited too long to... bring the case for jurors found that he was aware of this as early of 2021 and that open AI was shifting towards a for-profit structure meeting his 2024 lawsuit fell outside of the legal statute of limitation must has been seeking a hundred and fifty billion dollars in damages he also wanted all been removed from open AI is bored and hope to force the company back into a non-profit status. His legal team argued that OpenAI had stolen a charity and betrayed a co-founding principles. So there you go, what do you think? So, it's not that the jury disagreed with what he was saying. They were saying that it's the statute of limits. that was the ongoing issue. So what do you think here? It definitely makes it an interesting one. So can you have a nonprofit that wants to convert over to a profit without it being a issue? Because it definitely sounds like that is not the case. It definitely sounds like it is an issue if you opt to do that or attempt to do that. that in any means. So there you go. Okay, so let's continue talking about some other things that are going on. This one's been a while sitting around in regards of anything happening with it. That is for sure. So let's go ahead and talk about this one. So Luigi man Joni remember that name because that one was circulating for quite some time. a major courtroom setback after a judge ruled that the prosecutors can use the gun in the notebook allegedly tying him to the killing of Brian Thompson at trial. The judge decided that the key evidence found in Mangione's backpack, including a 3D-printed pistol, prosecutors say that matches the murder weapon, and a notebook describing the manifesto was legally obtained after the police later secured a warrant. Prosecutors say that the notebook included writings about wanting to target a health insurance executive and rallying against what he called the greed-fueled health insurance cartel. However, the judge also ruled that officers improperly searched the backpack during Mangione's initial arrest. arrest at McDonald's in Altoona, Pennsylvania, meaning that some items found during the first search, including a cell phone, passport, wallet, ammunition, magazines, and computer chips cannot be used in the trial. Mangione was arrested five days after Thompson was fatally shot outside a Manhattan hotel in December of 2024. Police say that Mangione's initially gave officers a fake name and a fake ID before eventually revealing his real identity. Prosecutors argued that police legal grounds to detain him while New York authorities moved on the case. Mangione had pleaded not guilty in both his state murder case and several other cases. federal charges. His a New York murder trial is scheduled to begin on September the 8th with a federal case expected to follow in October. So there you go sounds like a lot of man Joni going on as we are talking. What do you think here as we are talking about this because that's gonna be another thing that we are going to have to reference. No doubt about that. we are going. So there you go. There you go. Okay, take a listen to this one. NYC Health and Hospitals, a large public health provider in the United States has revealed that hackers stole sensitive data belonging to roughly about 1.8 million patients in a massive cyber attack. According to the officials, hackers quietly infiltrated the system through a compromised Sturge Barty vendor and remained inside the network from November until the breach was finally discovered in February. During that time, they reportedly copied huge amounts of deeply personal information, including medical records, diagnosis, treatments, insurance details, billing information, social security license data, financial account information, and even biometric data like fingerprints and palm prints. The breach is especially concerning because many patients in the system rely on Medicaid and the lack of health insurance entirely, making it particularly vulnerable to identity theft and fraud, unlike passwords, fingerprints, and biometrics. That's like a cybercrime equivalent of someone stealing your face and saying "good luck replacing that" unless you've seen that movie Face/Off. NYC Health and Hospitals say it has since reset credentials, tightened security controls, hired cyber security firms, and launched a continuing investigation. into the attack, patients are being urged to closely monitor bank accounts, insurance statements, and credit reports for suspicious activity and immediately change passwords tied to the compromised accounts. So what do you think there? I mean, I think it's definitely one of those interesting ones that if you're going to have this issue, and you got hacked and you know, it had an issue on 1.8 million people. Maybe those are the people that you have to give something to like maybe like some security packages in regards of somebody actually taking a look to make sure that nothing is happening to their stuff. I'm just saying at safety FM are not responsible for what this idiot behind the microphone He is trying to be entertaining. Welcome to our safety show. Hey, I'm just saying. So there you go. Let's do one more before we get into our main story. President Trump has agreed to drop a lawsuit against the IRS and the Treasury Department in exchange for the creation of a massive new $1.776 billion anti-weaponized fund designed compensate people who claim they were targeted by political motivated investigations. The lawsuit stemmed from the leak of Trump's tax returns, and also included complaints tied to the 2022 Mar-a-Lago raid and Russia election interference probe. Under the settlement, the new fund would be overseen by five members of the commission General, though Trump would retain the authority to remove the commission members. The agreement also allows the fund to issue both formal apologies and financial compensation to approved claimants. The administration says the fund is meant to help victims of what it calls government law lawfare. Lawfare. political weaponization. Acting Attorney General Todd Blanch defended the move, saying that the government should never be used against citizens for political purposes and that the process is intended to provide legal means. Democrats erupted over the deal, calling it an "unprecedented abuse of taxpayer money" and accusing Trump of effects. creating a politically controlled payout system, Senator Ron Wyndon described it as a slush fund while representing Jamie Raskin accused the administrating of funneling taxpayer dollars toward January 6 rioters and Trump allies. So there you go. That's gonna definitely be one of those ones that is not going away that one's not going away anytime soon. as we are talking. So there you go. Let's go ahead and get into some main story stuff is probably what we should be doing if you're ready. - Here is our main story on the Rated R Safety Show. - Okay, so let's do this. It's main story time. Time for you and I to hang out and strip away the corporate jargon. and talk a little bit about what the hell is actually going on inside this multiverse of ours. Today I wanna talk about the biggest lie currently circulating in corporate America and listen there. There are lies out there. It's bigger than that. We're like a family here. So it's bigger than what we offer the competitive salary. the legendary, the mythical, the absolute unicorn of the modern workplace. You ready for this? It's the open door policy. Think about it for a moment. How many times have you sat in an orientation or a town hall meeting or a safety stand down or some executive or a site manager stands up there and points to their offers and says, listen, let me let me Let me do that again. - Listen guys, my door is always open. - So listen guys, my door is always open. If you ever see something, say something, we can speak about the culture here. So it sounds like it's magical. I mean, it just sounds like the most magical thing, right? It sounds like you work in the utopian society where your thoughts and feelings. are actually valued. But let's be realistic for a moment, and just for a tiny moment, let's really pull back the curtain on this. Because in my experience, and in the experience of 1000s of other people who have reached out to me over the years, that open door policy is always a trap, and let me explain what I mean when I say that, when they say my door open there is an invisible asterisk at the end of that thing that the end of that sentence for sure and if you read the fine print my door is always open as long as what you have to say aligns perfectly with what I already believe doesn't cost me my any money and and and and doesn't point out the flaw in the I personally designed. Because the moment that you walk through that door, with actual systemic feedback, the moment that you walk in there and say, Hey, boss, this new procedure we rolled out last week is completely broken, and it's putting our guys in the in the line of fire. Or Hey, this safety initiative is just pencil whipping, it's not actually making anyone safer, watch how fast that open door suddenly feels like a brick wall. No, seriously. You see the organizations love the idea of feedback and they love the optics and the psychological safety. They want to put it on a poster and the breakdown right next to the stale. doughnuts on the labor law posters from 2018 but they don't actually want the friction that comes with a real unfiltered feedback. What happens to the guy who or gal who actually uses the open door? Let's call this person Bob. This is not Bob Edwards, just in case we love him around here. Bob is a good worker, Bob cares, Bob walks into the manager... manager's office and points out the legitimate safety hazards caused by a ridiculous production quota. Bob thinks that he's doing the right thing, and he's carrying the signal, right? But the manager, the manager doesn't hear the solution. The manager hears the problem. The manager hears insubordination. The manager hears a threat to their quarterly bonus. So what do they do? They smile. They say, "Thanks for bringing this to our attention, Bob. We'll look into it." And then the mutation happens. Suddenly, Bob is no longer the guy who cares about safety. Behind closed door, Bob is labeled a complainer. Bob is not a team player anymore. Bob is a resistance. to change. A few weeks go by and suddenly Bob is getting written up for being three minutes late from lunch. Suddenly Bob's performance review takes a massive hit because lacks positive attitude, and I'm just asking for a friend here. Yes, asking for a friend. Have you ever seen this happen? Like seriously, have you ever seen this happen? Have you ever watched somebody walk through that open door only to find out that it was a trap door with a target painted on the bottom of it? This is what we talk about when we discuss human and organizational performance, or HOP if you want to call it that way. A healthy system, a true learning organization, thrives on bad news. Yes, thrives on bad news. They want the bad news, they want it fast because bad news is the only way to fix a failing system before it kills somebody. But a toxic system, a toxic system treats bad news like a virus. It attacks the host that brought it in, and we see this in safety all the time. We all tell employees or team members to report near misses. We practically beg them to do this. Please, please, please, please fill out the cards. Tell us where the hazards are. But the second an employee fills out a card that requires management to actually pause production, spend capital or admit that leadership made a mistake, the system goes into full blown defense mode. The employees are investigated. Well, why are they just standing there? Why didn't they use their situational awareness? Why didn't you just make a better choice? We take systemic failure and we weaponize it against the worker. We package their voice. We rebrand their concerns. put it on the blame right back onto their shoulders, and then we wonder why six months later nobody's reporting anything anymore. We sit around with the boardroom scratching our head, "Gosh, our year misreporting is down 80%. We must be doing really safe things. This must be a really good safe workplace." No, you idiots, you don't have a safe workplace, you have a silent workplace. A silent workplace is the most dangerous environment on the planet, because the hazards are still there, the risks are still there, and broken systems are still there. The workers have just realized that your open door policy isn't worth a career suicide. if they walk through it. So here's the challenge to you today. If you are a leader, a manager, a safety professional, or just somebody with a title on your door, I want you to take a hard look at how you react when somebody brings you the ugly truth. Do you get defensive? Do you immediately start looking for ways? at the employee screwed up? Or do you politely nod and then bury their concerns in a filing cabinet somewhere? Because if you do, do me a favor. Matter of fact, do everyone a favor. Shut the door. Lock it. Just be honest about who you are and what kind of culture you're running and stop pretending that you want the truth if you're not willing to accept it, and if you're a worker out there, and if you're one of the ones that keeps trying to walk through the door and keeps getting slapped for it, I want you to know that you're not crazy. You're seeing the matrix for what it is. You are carrying the signal, and sometimes the system just isn't built to receive it, and it might well, it might be time to start doing the unspectacular planning that we talked about a few days ago. Because staying in the environment that punishes you for caring is a soul crushing way to make a living. I mean, think about that for a moment. I don't know. I don't know. But maybe I completely am off base here. But maybe I'm just an idiot behind this microphone but who the hell knows but I'm asking you one question because someone has to because if you don't think about these things and realize what is going on what are you gonna do next seriously what did she just say now back to real safety talk on safety FM. You're listening to the home of real safety dog. Safety FM. We'll be right back. Jay Allen, PhD, host of the Rated R Safety Show and founder of Safety FM, has spent years pulling back the curtain on the systems we work in, and the ones we don't talk about what started as a fatal crash on the 405 became a search for truth through behavior-based safety human and organizational performance simulation theory and everything in between safety in my opinion is raw reflective and real from burnout to breakthroughs from the matrix to moment someone says, "This doesn't feel right." Benjamin Franklin said those who would give up essential liberty to purchase a little temporary safety deserve neither liberty nor safety. That quote comes up often in the context of new technology and surveillance, but many experts agree that that's not really what he originally meant. Benjamin Franklin was a very smart man. He gave us inventions that changed the world at the time, but even he could not have imagined the world we live in today with computers, and phones in our pocket, and social media, and AI. I believe he would be even more worried, by the way, about these things. Now that's just a guess. I actually read his autobiography many years ago.
